PLEASE READ THE FOLLOWING AND CHECK THE BOX AT THE END TO INDICATE THAT YOU HAVE READ IT AND AGREE TO IT:

In the event my child becomes ill or injured while under SAINTS supervision, I/we approve authorities to take the following steps:

1. Contact a parent/guardian of the student and follow his/her instructions.

2. In the event neither parent(s) nor guardian(s) can be reached, contact the physician and follow his/her instructions.

STATEMENT OF RELEASE

I/We the undersigned hereby grant the above-named student(s) permission to participate in SAINTS of Florida, Inc. (SAINTS) sponsored activities. I/We release and hold harmless SAINTS and its staff from all liability for mishap or injury to the student(s) named herein from the time of drop off to the time of pick up. In the event my child(ren) need(s) medical/surgical services which require my consent before being applied and I/we cannot be reached, I/we hereby authorize, appoint, and empower a SAINTS representative to furnish on my/our behalf such written or oral authorization as may be required. It is understood the best possible care will be given to my child(ren).

COVID-19 WAIVER

I understand that there are many known and unknown factors involved regarding the COVID-19 virus and I agree that in the event that my child is exposed to or contracts COVID-19, I will not hold SAINTS S.P.O.R.T.S P.E. FOR KIDS or its staff liable.

I agree to uphold all guidelines, policies, procedures, and protocols that have been put into place by SAINTS for the benefit of all students and their families.

I give permission to have my child's temperature taken (on the wrist) prior to participation in SAINTS activities.

I agree to notify SAINTS if my child or anyone in our family tests positive for COVID 19 or exhibits any of the following symptoms and that my children will not participate in SAINTS activities until all symptoms have completely subsided.

Fever
Sore Throat
Dry Cough
Unusual Fatigue
Shortness of Breath
Runny Nose
Stomach Pain or Gastrointestinal Issues

I trust that all participants and staff will follow the above guidelines with the utmost of integrity and sincerity of heart in our desire to do the very best for one another and our families.

Photo Waiver

I, the undersigned also authorize the filming, photographing, and recording of my child(ren)s voice or likeness and the use of my child(ren)s first name for all lawful purposes in connection with the specific activities of SAINTS

** In rare cases, some parents prefer that their children not be photographed at Saints. If this is the case, please contact Coach Rick Andreassen for a photography addendum.
